About this Lunch & Learn
All the new roles are confusing! This Lunch and Learn package explores the reason for the new terms using the NHS film on Universal Personalised Care which is short and clear.
This resource then looks at the following roles and their benefits to the practice and patients:
- Active Signposters
- Care Co-Ordinators/Navigators
- Social Prescribers/Link Workers
- Health & Wellbeing Coaches.
The resource encourages the audience to think about their own practice and PCN (Primary Care Network) to see how these roles currently fit into their health system and how they might fit in going forward.
Certificates of attendance are provided as part of the package, and don’t forget to log your learning points & reflections as part of your CPD. CQC are particularly keen to see any evidence of your learning and how you apply it.
You can access the files of any purchased package as often as you like from your Wessex LMCs account. We routinely maintain packages to keep them up-to-date. We recommend, therefore, that you don’t save your own versions, as these will not be updated automatically.
